Job description
Aviata Health Group is seeking a Nurse Practitioner (Skilled Nursing) to lead internal medicine services during Saturdays and Sundays in a Skilled Nursing Facility. You will collaborate with the facility’s interdisciplinary team and physicians to reduce weekend hospitalizations and manage the entire patient population with nursing staff support.
The role involves evaluations for new admissions, discharge coordination, and care for conditions across acute and chronic illnesses.
