Job description
Hartford Healthcare is seeking a Weekend OASIS Registered Nurse to provide comprehensive in-home assessments and skilled nursing care to patients across our Connecticut service area. In this role, you will complete accurate OASIS assessments, develop and update individualized care plans, administer treatments and medications, and educate patients and families on managing conditions at home. You will collaborate closely with an interdisciplinary team, leveraging advanced technology and evidence-based practices.
