Job description
Carle Health in Illinois is seeking a Home Care RN/Field Case Manager to coordinate and deliver direct and indirect patient care in home settings, working with an interdisciplinary team for optimal outcomes. You will perform OASIS assessments, manage wound care including wound vac and catheters, and oversee IV infusions while guiding case management and caregiver communication across the care continuum.
