Job description
Carle Health is seeking a Home Care RN/Field Case Manager to join an interdisciplinary home care team. You will coordinate care plans, educate patients and families, and perform skilled nursing assessments.
Independent practice with opportunities to deliver high-quality home health visits across assigned counties, collaborating with LPNs, HHAs and therapists to ensure optimal patient outcomes.
