Job description
Northern Arizona Home Health, part of the LHC Group and Optum family, is seeking a Registered Nurse to provide and coordinate home health care for patients in their homes as prescribed by physicians and in compliance with laws and agency policies.
Responsibilities include developing care plans, coordinating with the interdisciplinary team, supervising aides and LPNs, and timely documentation of patient care.
