Job description
A nonprofit organization is seeking a Wellness Coach in the Bronx, NY area to provide case management, community outreach, and hands-on support to adults and families with behavioral health needs.
About the Opportunity:
Schedule: Monday to Friday
Hours: Standard business hours with flexibility to accommodate later field visits, as needed
Setting: Onsite
Responsibilities: Provide case management services to an assigned caseload of residents and families
Coordinate referrals and follow-up for medical, behavioral health, substance use, vocational, and public benefit services
Conduct home and field visits to support clients in the community and maintain continuity of care
Assist clients with daily living skills, supportive counseling, crisis intervention, and advocacy
Document services accurately and collaborate with the care team to support housing stability and community integration
Qualifications: Bachelor's Degree
Prior case management experience
Experience working with adults and families in behavioral health, mental health, substance use, or supportive housing settings
Ability to work independently, stay organized, and manage a community-based caseload
Strong written and verbal communication skills
Desired Skills: Bilingual (English and Spanish)
Experience working in a scattered-site housing setting
Familiarity with the AWARDS system
Experience supporting clients with substance use and mental health needs
Knowledge of community resources, medical follow-up, and public benefits coordination
