Job description
Northshore Home Health Care is seeking a Wellness Nurse for an assisted living and memory care community. You will provide daily nursing care, medication management, and health assessments, while collaborating with families, caregivers, and physicians to tailor care plans.
Responding to changes in residents' conditions with safety and dignity, you will build meaningful relationships and foster a warm, home-like environment with a patient-centered approach.
