Job description
Four Seasons is seeking an RN Case Manager to coordinate nursing plans of care across a ~15 patient caseload in Macon County and surrounding areas. You will supervise LPNs and Hospice Aide staff, deliver direct nursing care, and work with the Clinical Team Leader to ensure high-quality patient outcomes.
The role requires NC RN licensure, strong communication, and proficiency with EMRs. Monday–Friday with some on-call coverage; full-time, part-time, or PRN options may apply.
