Job description

Gateway Wound Care (GWC) is seeking compassionate, clinically skilled Advanced Practice Providers (Physician Assistant, Nurse Practitioner) to provide mobile advanced wound care and primary care throughout the St. Louis metropolitan region.

This is an intentionally part-time launch opportunity, beginning with up to two clinical days per week and designed to grow alongside Gateway's patient volume and referral network.

Providers will care for patients where they receive care-including private residences, skilled nursing facilities, assisted living communities, and other community-based settings-while working closely with Gateway's physician leadership and interdisciplinary care team. Our focus extends beyond the wound itself to the underlying clinical and social factors that influence healing, with the goals of improving outcomes, preserving limbs, strengthening continuity of care, and preventing avoidable hospitalizations.

Gateway handles the backend-including referral intake, scheduling, geographic routing, billing, credentialing, and care coordination-so our clinicians can focus on what they do best: caring for patients and achieving strong clinical outcomes.

The ideal candidate is clinically confident, collaborative, comfortable working independently in the field, and believes that healing requires more than treatment alone- it requires continuity, coordination, compassion, and attention to the whole person.

What You'll Do

Care for a growing panel of patients through efficient, geographically optimized routes coordinated by the GWC team.

Perform comprehensive wound assessments, evaluations, and ongoing management of acute and chronic wounds.

Complete timely, accurate, and compliant electronic documentation for all patient encounters within Gateway's EMR.

Perform bedside procedures within scope of practice, including:

Sharp and selective debridement

Dressing selection and application

Compression therapy management

Offloading recommendations

Negative Pressure Wound Therapy (NPWT) management

Skin substitute and advanced wound product application (if credentialed/trained)

Identify risk factors for delayed healing, hospitalization, limb loss, and other wound-related complications requiring escalation of care.

Promote limb preservation, functional independence, and long-term health outcomes through proactive intervention and preventive care strategies.

Provide home-based primary care pertaining to chronic disease management, medication management, and routine follow-up (diabetes, CHF, COPD, and related conditions).

Develop individualized treatment plans based on patient needs, comorbidities, and evidence-based wound care practices in collaboration with Gateway's physician leadership

Work alongside Gateway's care coordination team to address barriers to healing and facilitate appropriate specialty care and community resources

What We Offer

$550 per 6-8 hour clinical day, up to two clinical days per week initially (guaranteed). No nights, weekends, or on-call.

$1,250 signing bonus

Flexible, part-time schedule with room to grow as patient volume and Gateway's clinical footprint expand

Company vehicle option available

Full administrative and referral support-including intakes, scheduling, billing, credentialing, care coordination and communication, referrals, and geographically optimized routing-so you can focus on patient care

Physician collaboration and clinical oversight with an interdisciplinary, patient-centered team

Opportunity to build meaningful patient relationships while helping shape an innovative community-based care model from the ground floor

Qualifications

Required

Current Missouri license and authorization to practice as a Nurse Practitioner/APRN or Physician Assistant

Current DEA registration, or ability to obtain if required for the role

Valid driver's license and reliable transportation

Ability and willingness to travel throughout an assigned St. Louis metropolitan service area

Strong clinical assessment, procedural, communication, and organizational skills

Comfort providing care autonomously in community-based settings while collaborating closely with physician leadership and an interdisciplinary team

Ability to provide consistent part-time clinical availability during Gateway's launch

Preferred

Missouri and Illinois licensure

Wound care certification or advanced wound training (WCC, CWS, CWCN, CWON, or similar)

Experience in wound care, primary care, geriatrics, hospital medicine, vascular medicine, home health, palliative care, skilled nursing/post-acute care, or other medically complex populations

Experience with bedside wound assessment and debridement

Experience with Medicare documentation and mobile/home-based healthcare delivery

Comfort working within a growing, entrepreneurial healthcare environment

Comfort with a 1099 arrangement, if applicable to the final engagement structure

Previous wound care experience is preferred but not required. Providers with strong backgrounds in primary care, geriatrics, hospital medicine, home health, palliative care, vascular medicine, or post-acute care are encouraged to apply.
