Job description
Hartford HealthCare at Home is seeking a Wound Program Specialist to develop and coordinate statewide wound care initiatives in patients receiving home care. You will provide expert consultation, hands‑on care, and standardized treatment protocols to improve outcomes across the agency and community partners.
The role requires an RN with a WOCN certification and at least two years of specialized home care nursing.
