Responsibilities
Assess and document wound status and progress during on‑site visits.
Develop and implement individualized wound care plans with measurable goals.
Provide evidence‑based wound care treatments and dressing changes.
Adhere to infection control protocols and patient safety standards.
Monitor and document patient outcomes to track pipeline and targets.
Coordinate with interdisciplinary team to optimize care continuity.
Ensure compliance with regulatory standards and facility policies.
Maintain accurate patient records and secure reporting of progress metrics.
Demonstrate accountability for personal performance and meeting targets.
Educate patients and families on wound care and self‑management.
Identify opportunities for professional development and skills advancement.
Experience/Qualifications
Active RN license with wound care certification preferred.
Proven clinical expertise in wound assessment and dressing changes.
Ability to develop and implement measurable wound care plans.
Strong documentation and electronic health record accuracy.
Infection control and patient safety protocol adherence.
Excellent coordination with interdisciplinary team for care continuity.
Knowledge of regulatory standards and facility policies.
Track patient outcomes and pipeline metrics for performance visibility.
Strong communication skills for patient and family education.
