Job description
yeshouse is seeking a dedicated Case Manager for the Independent and Transitional Living Program to support at-risk youth ages 14–24 in achieving safe, stable, and independent futures.
You will work under the Program Coordinator to develop personalized plans, connect resources, and teach life skills, while documenting progress in HMIS and related systems. This role requires strong organization, communication, and professional boundaries.
