Job Summary
The Youth Case Manager provides care coordination and case support to children and adolescents. The Case Manager ensures every individual's comprehensive needs are met and they are able to access social, medical, educational and other services. The Case Manager assesses the individuals and their families' needs, shares resources, makes referrals, and engages with community support. The Case Manager works with a team that supports the goal of moving the individual and their families toward greater self-reliance. The Case Manager will also serve as a liaison with other service providers on behalf of the individual. This position reports to the Youth Case Management Coordinator.
Job Responsibilities
Conduct a comprehensive case management assessment to determine individual strengths, needs, resources and support systems
Work with individuals to identify and prioritize needs and establish goals for service
Serve as referral/liaison between individuals and other internal and external treatment programs
Research and coordinate internal and external resources; link individuals with community providers to provide collaborative services
Make referrals to connect individuals to needed resources and services
Actively participate in training and case consultation sessions with treatment team including presentations of cases
Maintain complete, accurate and current clinical charts
Other Duties, as assigned
Requirements
Job Requirements
Essential Must pass all required Background Checks.
Must possess a valid Virginia Driver's License with reliable transportation and driving record acceptable to agency insurance carrier.
Must obtain a TB test with satisfactory results within 30 days of employment, and yearly thereafter as required by licensing authority.
Education and Experience
Bachelor's Degree in Human Services, Social Services, Psychology, or related field.
Experience
1 to 2 years' experience in case management, or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
