Job description
EAC Network is seeking a dedicated full-time Case Manager for the Alternatives for Youth (AFY) Program at 320 Carleton Ave, Central Islip, NY. The role focuses on home-based crisis intervention and comprehensive case management for families involved in PINS proceedings, with bilingual English/Spanish support essential.
You will assess needs with the CANS tool, develop intervention plans, make referrals, and maintain accurate records while coordinating with partner agencies.
