Job description
Southeast Healthcare Services in Columbus, OH is seeking a dedicated Transition Youth Specialist to provide community-based services to youth ages 14-18. The role focuses on helping participants develop independent living skills and employment readiness.
You will conduct assessments, develop individualized service plans, and coach youth toward employment while coordinating with families, service providers, and systems of care to support successful transitions to adulthood.
