nAdditional Information:

nThe Case Manager Assistant works with the case managers to facilitate patient's continuity of care for home at time of discharge. The Assistant is responsible for making referrals to community care agencies and communicating these arrangements with the case manager and documenting in the patient's medical record. Additional duties include assisting in writing appeal letters for denied medical stay and completing discharge follow-up phone calls.

nDescription

nnnComplete referral to community resource agencies for patients with home discharge needs.

n

nnCoordinates and communicates the arrangement of the discharge planning activities.

n

nnDocuments agencies who will be providing services in the medical record along with the completion of follow-up phone calls.

n

nnCompletes discharge follow-up phone calls and documents in medical record. Communicates any relevant gap in providing care to a patient that is identified during the phone call.

nnAssist the manager in writing appeal letters for denials of in-patient admissions.
