Case Manager
The Case Manager coordinates patients through the acute care hospitalization and manages all aspects of discharge planning and transition process. The role also performs utilization management and quality screenings, with a focus on in-patient case management.
Responsibilities:
Coordinate patients through the acute care hospitalization.
Manage all aspects of discharge planning and transition process.
Perform utilization management and quality screenings.
In-patient case management is required.
Requirements:
BSN required.
NJ or Compact state RN license.
Required Skills:
2 years of acute care experience strongly preferred.
Experience in case management and managed care.
In-patient case management is required.
Hospital experience, including discharge planning and placement in rehabilitation facilities.
Minimum 2 years of experience with acute inpatient hospital discharge planning.
Experience with subacute patients required.
