Number of Positions in Requisition1Job DetailsThe Clinical Pharmacist oversees the medication distribution system for their area of responsibility. In addition, the Clinical Pharmacist identifies, prevents, and resolves drug therapy problems by ensuring each medication is indicated, effective, safe, and convenient for the patient.Job DescriptionJob DutiesInteracts with co-workers, visitors, and other staff consistent with the values of Jefferson.(Do not delete, move or over-write this statement)Assures the appropriateness of drug therapy and compliance with hospital policiesOversees the distribution of medications throughout TJUHSupports implementation of processes to assure the quality of Pharmacy servicesTrains staff as assigned (e.g., residents, interns, technicians, new pharmacists)Precepts student pharmacists as assigned. See Preceptor Development & Evaluation Programpolicy.Minimum QualificationsDoctorate Doctor of Pharmacy or RPH (see below under experience)Knowledge of pharmacokinetic principles required for medication dosingMonitor patients for response to medicationsIdentify and document sentinel/serious events, near misses, medication errors and adversedrug reactions in the electronic reporting systemPromote and assure compliance with the hospital formulary and policies and proceduresdefined by the P & T Committee.Provide care that is appropriate to the chronological age, developmental age, social maturity,and cultural background of the patientRP - Registered Pharmacist - State of PennsylvaniaPreferred Qualifications1 year PGY1 Pharmacy Residency preferredMicrosoft Word and Microsoft Outlook skillsAdditional QualificationsCertification required based on primary work location.Physical DemandsLift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.Job Description Disclaimer: This position description provides the major duties/responsibilities, requirements and working conditions for the position.
