Job description
Activate Care is seeking a Clinical Support Supervisor to provide clinical guidance and escalation support to Community Health Workers delivering the Utah LIFT Path Assist for Chronic Disease. This role ensures CHWs can support patients with diabetes, hypertension, obesity, COPD, asthma, and cardiovascular risks while staying within non-clinical CHW scope.
