CNA, Full-time
Transitions LifeCare is hiring immediately. As one of the community’s industry leaders, we provide exceptional home health, hospice, and palliative care services throughout our 8-county territory. Transitions LifeCare’s goal is to care for individuals and families with compassion, support, and expertise as they navigate the end of life or life changing illness.
Hiring for the Following Counties:
Wake
Durham
Johnston
Franklin
Harnett
Orange
Chatham
The schedule for this position is Monday-Friday 7:00am-3:30pm; rotating weekends and holidays.

A Successful Certified Nursing Assistant (CNA) has the following: Caring and compassionate attitude with true concern for people
Strong customer service focus
Ability to work collaboratively as part of a team
Ability to multitask
Responsibilities: Provide high-quality and compassionate care to patients per their individualized care plan
Works collaboratively with the interdisciplinary team (IDG) in achieving patient care goals
Maintain comfort, privacy, and dignity of our patients
Promptly reporting pertinent information to the immediate supervisor
Requirements: Current CNA listing with NC Home Health Aide Registry
Must have reliable transportation
Hospice or home health experience preferred
Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines

Complete New-Hire Orientation and clinical skills training
Receive continuous training from our team as you continue to provide exceptional care to our patients
