Job description
Gentiva Hospice is seeking a compassionate RN Case Manager to provide bedside nursing and care coordination in patients' homes or designated settings in Kansas. You will assess, plan, implement, and evaluate nursing care guided by each patient’s Plan of Care, while supporting families and the broader care team.
Work with an interdisciplinary team to update care plans, educate caregivers, and ensure timely documentation.
