Description
Assess patients in both inpatient and outpatient environments to formulate personalized treatment plans
Deliver physical therapy treatments emphasizing mobility, strength, balance, and functional restoration
Engage in collaboration with nurses, physicians, case managers, and other rehabilitation experts to enhance outcomes
Inform patients and their families about therapeutic objectives, discharge procedures, and at-home exercise routines
Maintain thorough documentation of evaluations, treatments, and progress according to facility policies and legal standards
Take part in care team meetings and interdisciplinary discussions to ensure seamless patient care
Education
Doctorate in Physical Therapy (DPT) or a comparable qualification from an accredited institution
Certifications
Active state licensure as a Physical Therapist
CPR certification preferred
Additional certifications (e.g., neurological, geriatric) are advantageous but not mandatory
Skills
Prior experience strongly preferred
Ability to address complex medical and mobility challenges among patients
