Job description
Southwest General Home Health is seeking a Registered Nurse Case Manager to lead patient assessments, plan of care, and coordination across disciplines under physician direction.
The RN educates patients and families, ensures safety and accurate documentation, and remains compliant with state rules and regulations. Requires active RN license and at least one year of acute care experience; home care experience is preferred.
