Job description
Ravenswood Family Health Network is seeking a physician to deliver outpatient primary care within a PCMH framework. You will work with other providers to define best clinical practices, document data for quality improvement, and ensure care meets established protocols while supporting wellness, prevention, and chronic disease management.
Include board certification and excellent communication skills; ability to work with a culturally diverse team and patient population; RFHN uses Epic.
