Job description
21st Century Home Health Services (21HHS) seeks an entry-level Health Field Physical Therapist for the Davis area. You will build core clinical skills in a home health setting, focusing on follow-up visits under supervision.
As part of an interdisciplinary team, you’ll perform patient evaluations, implement plans of care, document care within 24-48 hours, and participate in weekly case conferences. CA license and driver’s license required.
