Job description
Alpha Home Health and Hospice is seeking a Family Medicine Nurse Practitioner or Physician Assistant to provide comprehensive outpatient primary care in Minnesota. The role supports preventive, acute, and chronic care, with a focus on improving patient outcomes and the patient and provider experience.
We offer a full-time, day-shift schedule with 0.6–0.9 FTE and 3- or 4-day work weeks, no evenings or weekends, and potential bonuses.
