Job description
PruittHealth Corporation in Georgia seeks a Nurse Practitioner specialized in D-SNP to provide holistic care, conduct home visits and telehealth health risk assessments for new and transitioning members. You will collaborate with physicians, RNs, case managers and families to diagnose, manage acute and chronic conditions, coordinate transitions, and document care plans within our care-management framework.
Requirements include NP or PA licensure, AANP/ANCC certification, CPR, and 2+ years
