Job description
The Chatham Hills Subacute Care Center is actively seeking an experienced Physical Therapist to provide direct patient care and rehabilitation planning for residents. The PT will assess patients, design and supervise treatment programs, set achievable goals under physician guidance, and document patient progress while educating families in therapy methods.
Geriatric/long-term care experience and solid documentation skills are preferred.
