Job description
Duties
Help
Conduct community outreach in support of population health.
Identify high-risk units with suboptimal population health metrics.
Evaluate population health interventions in order to assess impact, continuously improve services, and derive lessons learned.
Serve as installation subject matter expert in effectively implementing health communication interventions.
Integrate and implement community outreach and prevention programs through actively participating in the installation Community Action Team.
