nJob Description:
nnProvide direct patient care in a home setting, ensuring the comfort and well-being of patients
nAssit patients with personal care (bathing, grooming, dressing)
nHelp with mobility and transfers
nEvaluate home environment for hazards or barriers to more independent living.
nMonitor vital signs and report changes to nurse/ supervisor
nEducate and instruct patients, family members, or other patient representatives, in safety
nAccurately document observations, interventions, and evaluations pertaining to patient care management and services provided.

nRequirements:
nnValid Home Health Aide (HHA) in the state of California
nMinimum 1 year of experience as an HHA in Home Health
nAbility to lift/move patients safely
nProficient in taking and interpreting vital signs
nStrong communication and interpersonal skills
