Job description
Choice Health At Home is seeking a Home Health Field RN to provide comprehensive in-home nursing care in the Sapulpa area. This full-time role involves assessment, planning, and evaluation of patient care, coordinating with families and the care team to promote independence and better quality of life.
The RN will perform admissions assessments, develop care plans, educate patients and families, and supervise aides and LVNs/LPNs as needed.
