Job description
Interim HealthCare is seeking a Home Health Physical Therapist to deliver rehabilitation services in patients' homes across McHenry, IL. You will assess mobility, guide exercise programs, and coordinate with a multidisciplinary team to achieve optimal outcomes.
The role requires a Graduate of an accredited PT program with an active Illinois license, CPR certification, and a passion for in-home care. Expect flexible assignments, 1:1 therapist-to-patient ratios, and ongoing training.
