Job description
Assured Home Health, part of LHC Group within the Optum family, is seeking a Physical Therapist to join our home health team. You will conduct assessments, develop treatment plans, and deliver interventions to improve mobility and function in patients at home.
Responsibilities include evaluating functional status, supporting the plan of care with the care team, and documenting progress while complying with state guidelines and company policies. Oregon licensure is required for this role.
