Job description
AdventHealth is hiring a licensed Physical Therapist for home health in Florida. The role develops and revises patient-specific care plans, educates patients and families, and coordinates care across caregiving teams in a home setting.
You will reassess progress, document outcomes, and ensure adherence to best practices while supporting the growth of others through mentoring. On-site work with daytime hours, potential on-call shifts.
