Job description
Altenheim Home Health Care seeks a Physical Therapist to provide rehabilitative care in patients' homes within Lorain, OH. You will work as part of an interdisciplinary team to implement the Plan of Care and promote patient independence.
The role requires a state license in Ohio, a valid driver's license, and at least two years of PT experience. Extensive in-home assessments and patient-family education are part of daily duties.
