Job description
Ascension at Home is seeking a licensed Home Health Physical Therapist to plan, implement, and evaluate patient care plans to restore or maintain well-being. The clinician will assess patients across ages, perform OASIS assessments when appropriate, and collaborate with an interdisciplinary team to ensure coordinated care.
