Job description
BAYADA Home Health Care in Fort Wayne, Indiana is seeking an experienced Physical Therapist (PT) for a full-time role delivering home health visits to adult and geriatric clients. Responsibilities include evaluating patients, developing care plans, performing mobility and functional assessments, documenting care using EMR technology, and collaborating with a multidisciplinary team to support independent living at home.
One year of licensed PT experience is required; Indiana license is required.
