What You'll Do * Provide Skilled PT Care: Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients. * Implement Therapeutic Interventions: Focus on functional strengthening, gait training, balance, and fall risk reduction to improve patient mobility and independence. * Manage Your Caseload: Schedule and conduct home visits based on your availability and patient needs. * Collaborate for Optimal Outcomes: Work with a supportive care team, including other disciplines and supervisors, to ensure coordinated patient care. What You'll Need * Valid California Physical Therapist license * CPR or BLS certification * Driver's license and auto insurance * Physical + TB test (within the last year) * Professional liability and business license (if applicable) Where You'll Be Working We're looking for a home health clinician to cover Santa Clara, including San Jose, Sunnyvale, and Mountain View.
