Job description
CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to join our caring team. You will work onsite in patients' homes, implementing the physical therapy plan developed by the supervising Physical Therapist and helping patients regain strength, mobility and independence.
You will document client records weekly, participate in department in-services, ensure infection control, and coordinate with the Physical Therapist and Branch Director.
