Job description
Adaptive Home Health is seeking a Physical Therapist to work in a field-based home health role serving Baytown and surrounding areas. You will own the caseload, perform in-home assessments, and deliver skilled PT visits while coordinating with a multidisciplinary team.
You will build and execute a plan of care focused on gait/balance, transfers, strengthening, and functional mobility. A Texas PT license and willingness to travel daily are required.
