Job description
CenterWell Home Health is seeking a licensed Physical Therapist to deliver in-home therapy, helping patients regain strength, mobility and independence through personalized plans focused on functional improvement. In this patient-facing role, you will supervise PTAs and Home Health Aides, document progress, coordinate with the care team, and report to the Clinical Manager.
