Job description
UnitedHealth Group is seeking an RN Case Manager in Massachusetts to guide patients and families through the care continuum, assess needs, coordinate care, support discharge and transition planning, and manage ongoing outpatient care. The role includes collaboration with a Care Advocate and Social Worker, under supervision of an RN or MD.
The position involves patient assessments, referrals, teaching, supervision of LPNs and HHAs, and keeping thorough documentation.
