Job description
CenterWell Home Health, part of Humana, seeks a Home Health Registered Nurse to provide one-on-one skilled nursing care in patients' homes and to manage recovery plans in Hauppauge, NY. You will work with physicians and clinicians to support independence and return to daily life.
Responsibilities include developing care plans, monitoring patient progress, educating families, maintaining records, and supervising aides. A valid driver’s license and Humana driver-safety rules apply.
