Job description
Rochester Regional Health is seeking an experienced Registered Nurse (RN) to join our Community Health Home Care team in Lakeville, NY. The role focuses on in-home nursing interventions, patient education, and care management to promote good health and one-on-one care in patients' homes.
Responsibilities include comprehensive assessments, care planning, documentation, and collaboration with the interdisciplinary team. Per diem, per need schedule, with flexible days and potential incentives.
