nSchedule: Full Time
nShift: Monday - Friday 8:30am-5pm
nLocation: This is a fieldwork position covering Volusia and Flagler Counties.
nnnProvides personal care to patients according to the assignment details and care plan.
n
nnDocuments patient care and observations timely and appropriately.
n
nnParticipates in end-of-life care for patients and families. Informs the registered nurse of any changes in the patient's condition.
n
nnFollows the schedule of care, including making several visits per day with travel.
n
nnAssists patients with activities of daily living, such as bathing, dressing, and grooming.
n
nnMonitors and records vital signs, including temperature, pulse, and respiration.
n
nnSupports patients with mobility and exercise as prescribed. Provides emotional support and companionship to patients. Maintains a clean and safe environment for patients.
n
nnCommunicates effectively with patients, families, and healthcare team members.
n
nnOther duties as assigned.

nnGood phone etiquette (Required)

nnCritical thinking skills (Required)

nnKnowledge of TEAMS meetings (Preferred)

nnKnowledge of EPIC patient software (Preferred)

nnKnowledge of telephone protocol and ability to operate a multi-line phone system (Preferred)

n nEducation:
nnHigh School Grad or Equiv (Required)

n nWork Experience:
nnnExperience in the health care field (Required)

nnHospice experience (Preferred)

n nAdditional Information:
nnHospice Aide training from an approved 75 hour training course and ongoing training(Required)
n nLicenses and Certifications:
nnnBasic Life Support – CPR Cert (BLS)(Required)

nnDriver's License (DL) (Required)

nnAuto Insurance (AL) (Required)

nnCertified Nurse Assistant (CNA) (Required)
