Job description
AdventHealth is seeking a Field Hospice Nurse to provide nursing services to patients at home or in nursing facilities across Adams, Arapahoe, Jefferson, Denver, Douglas, Elbert and Elizabeth Counties. The role includes care plan initiation, coordination of CNAs, patient assessments, and family education.
The ideal candidate holds an Associate's in Nursing (RN) with BLS and demonstrates strong communication, documentation, and teamwork skills to support end-of-life care in diverse settings.
