Job description
Provide advanced, patient-centered care to people with serious illnesses, including hospice and palliative care. Conduct assessments and required hospice face-to-face encounters, manage symptoms and medications, coordinate care with physicians and interdisciplinary teams, and support patients and families with goals-of-care and end-of-life decisions across home and facility settings.
