Job description
Elara Caring seeks a Registered Nurse Case Manager to join our hospice/home health team. This salaried role focuses on coordinating patient care in home settings, assessing needs, and guiding families through the Interdisciplinary Plan of Care to ensure high-quality outcomes.
You will report to the Clinical Team Manager, travel up to 50% to patient homes, and collaborate with physicians, aides, and social workers to deliver the Right Care at the Right Time for patients in their preferred setting.
