Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to provide in-home health evaluations for patients in their homes. The visit lasts about 45 minutes and includes medication and medical history review, physical evaluation, and optional diagnostic tests.
Clinicians will use an iPad and a clinical workflow during evaluations and are not required to prescribe medications or order labs.
