Job description
Signify Health, part of CVS Health, seeks a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations in members' homes. Visits last about 45 minutes and include history review, physical examination, and any ordered diagnostic tests. Occasional virtual visits are also required.
You will travel locally and within licensed states, joining a national clinician network dedicated to bridging gaps in care and connecting patients to the right care.
